Every synthesis has a boundary
An evidence review reflects sources available through a stated cutoff. Material published afterward may not be represented, even when the earlier review was rigorous.
Source checks and human review differ
Source monitoring can identify candidates for review. It cannot determine whether a new publication changes a conclusion. That requires verification and designated human editorial approval.
Revision records preserve context
A useful revision history explains what changed and whether the update affected a conclusion, citation, status record, or presentation. That context makes maintenance visible without implying real-time completeness.
